During an Emergency, Information Matters!

The Envelope for Life contains your medical information and history, including medications, allergies, past illnesses, and emergency contact information. Simply complete the questionnaire, seal in the small waterproof envelope, and place inside your refrigerator. You then place a special magnet on your refrigerator that tells paramedics they can find your medical information inside the refrigerator. Southwest Ambulance paramedics are trained to look for this magnet when they arrive at your home.
